Vital Records

Vital Records are Birth and Death Certificates for events that occur in Madison County. Death information is collected from sources such as newspaper obituaries, news reports of accidents involving death, and notifications from hospitals and funeral homes. It is the responsibility of the parent to apply for a birth certificate when a home birth/delivery has occurred in Madison County. The Deputy Registrar is responsible for obtaining and preserving documents of all births and deaths in Madison County. This evidence protects the legal rights of individuals and provides a sound statistical basis for health and social planning. Births and deaths have been registered in the State of North Carolina since 1913. Prior to that, the State did not use birth records or certificates of any kind. In North Carolina, records are maintained in the Register of Deeds office in the County where the event occurred. If you are conducting genealogical research, contact the Madison County Register of Deeds for more information. Certificates or information from other counties or states must be requested from the place of birth or death.

Vital Records Services Include:

  • Help with preparing birth certificates
  • Help with preparing death certificates
  • Help with preparing paperwork needed for children born out of wedlock.  This is referred to as an “Affidavit of Parentage” (DHHS 1660)

Hours of Operation:

  • Monday-Friday 8:30am -5:00pm No appointment is necessary, however calling ahead is recommended.
  • Please bring a driver’s license or other picture identification and Social Security Card.
  • There is no charge for registering a vital event at the Health Department.  However, the Register of Deeds does charge for certified copies.
