Vital Records
Births and deaths have been registered through the State of North Carolina
since 1913. Prior to that, the state did not use birth record or certificates of any kind. If you are conducting genealogical research, contact the Madison County Register of Deeds for more information.
The Madison County Health Department processes all birth and death certificates in the county. If a birth occurs at home, the health department will record the home birth. Health department staff works directly with hospitals, physicians and medical examiners, and funeral homes.

The Deputy Registrar/Vital Statistics position processes certificates for all births and deaths that occur in Madison County. Deaths are collected from sources such as newspaper obituaries, news reports of accidents involving death, and notifications from hospitals and funeral homes. It is the responsibility of the parent to apply for a birth certificate when a home birth/delivery has occurred in Madison County.
The Deputy Registrar is responsible for obtaining and preserving documents of all births and deaths in Madison County. This evidence protects the legal rights of individuals and provides a sound statistical basis for health and social planning. Vital Records Services include:
o Help with preparing birth certificates
o Help with preparing death certificates
o Help with preparing paperwork needed for children born out of wedlock. This is referred to as an “Affidavit of Parentage” (DHHS 1660).
