WELCOME TO PATRIOT PLACE HEALTH CENTER
What an exciting time! Patriot Place Health Center is the first
school-based health center in Madison County and we are ready for business. We look forward to providing your child with the best possible health care.
Students perform better when they show up for class, healthy and ready to learn. School-based health centers bring the doctor’s office to the school so students avoid health-related absences and get support to succeed in the classroom.
Open during school hours, the health center will offer a wide variety of services that include, but are not limited to:
√ Age appropriate well-child exams √ Immunizations
√ Diagnosis and treatment for acute illness and injury
√ Management of chronic health conditions
√ Basic laboratory services
√ Health education √ Anticipatory guidance
√ Behavioral risk assessments and counseling
Patriot Place Health Center is operated by the Madison County Health Department which has partnered with Madison County Public Schools to provide services to Madison Middle School and Brush Creek Elementary School students. It will be staffed with a registered nurse, administrative staff, part-time nurse practitioner, and part-time psychologist.
Students are seen regardless of their ability to pay. The center will also bill Health Check/Health Choice, private insurance and offer a sliding fee scale based on family income.
PLEASE NOTE: Prior permission is required from the parent or legal guardian of any child seen at the health center. Enrollment forms are required. Complete them and return them to the school. We encourage parents to visit or call Patriot Place for any questions or concerns.
