Diabetes Self-Management Program

 

We want you to get ready to be in control of your diabetes!  The Madison County Health Department is now offering the diabetes self-management program.  This program will present information on monitoring, meal planning, medications, complications, sick days, exercise, and stress management.  The program begins with an initial assessment and continues with an all day group class and a three month follow-up.  Participants will benefit from education provided by nurses and a nutritionist, as well as other specialists when available.  The diabetes self-management program is for those with type I, type II, glucose intolerance and pre-diabetes.  A physician referral is required.
