Community Health Assessment

 

Community Health Assessments (CHA) are conducted every four years in collaboration with the Madison Community Health Consortium. The health assessment provides useful information from local community members about their health concerns, access to health care issues, and individual health status. A wide range of community health data is evaluated also. This information is used to steer the work of the health department and the consortium in addressing the community’s health concerns and monitor progress towards the 2020 Healthy Carolinians’ goals.
